Whitmire is a trusted resource and referral partner for legal professionals in charge of complex clinical care plans for clients.

  • We assist Special Needs Trust beneficiaries with a benefits analysis, budgeting for immediate and long-term care, and alignment with the client’s end-of-life wishes.

  • We help parents with children turning 18 apply for benefits and develop an independent living plan. Our team can also be a part of a solution to the least restrictive alternative to guardianship. If needs arise that are outside your area of expertise, we can help. 

  • We provide expertise in care planning, plus available community providers, resources, and benefits. Our team can also provide crisis support after a catastrophic injury.

Health Care Professionals

The list of healthcare providers we receive referrals from, including doctors, therapists, and social workers, is as diverse as our wide variety of clients. 

As clients transition out of medical facilities, we’ll work with you to plan, coordinate, and manage the care they will need to stay on track in the days, months, and years ahead. We can attend appointments, provide home visits, set up systems to maximize independence, coordinate care, facilitate direct care staff to help manage the day-to-day needs in someone’s home, and provide enrichment/engagement options. 

  • We support residents who live at home or in a facility (CCRC, Independent Living, Assisted Living, Memory Care, etc.) providing advocacy, clinical care coordination, boots on the ground security and support for adult children who live out of state. We can manage crisis support, home care staffing, provider realignment and enrichment/engagement services.

  • We provide support for clinical care coordination, home care staffing, independence skills development, enrichment/engagement services, transitions into college, guardianship plans and much more.

  • We support patients needing additional assistance with care needs at discharge or when transitioning to a care facility.

Fiduciaries

If you act on behalf of a client to manage finances and assets, we’re here to help. We often consult on future care needs and benefits.

  • We serve as a liaison between clients and other interests to create budgets, ensure bills are paid on time, secure benefits and manage the day to day finances.

    We assist in developing a care plan ensuring a client’s preferences, care needs, benefits, personal needs and quality of life are considered and within budget. 

    We provide wellness visits to ensure a client is living in a safe place with ancillary services if needed.  

    We educate fiduciaries to better understand how it may be best to work with their client. We offer a different perspective. Clients are misunderstood quite often; a care manager can help a fiduciary better serve the beneficiary, alleviating headaches on both sides to build a trusted and efficient working relationship. 

  • Whitmire will help clients manage a personal budget, provide money management skills training, and develop systems that help increase autonomy with money while ensuring financial safety.

  • Whitmire will serve on or act in a consultant role when there is a need to understand future care needs and available benefits or to resolve conflicts.
