Our Consultation Series is ideal for families or individuals seeking short-term, focused guidance. We typically meet for one to three sessions to help you navigate specific questions, transitions, or planning topics.

Consultation Series

This series is ideal for families or professionals who need targeted planning support around a specific issue, whether you're navigating a transition, making long-term decisions, or preparing for what's ahead.

Care Management: Onboarding & Initial Support

A structured, hands-on start to stabilize care and build a plan that works.

This service is ideal for individuals and families who are just getting started with care management and need immediate structure, clarity, and coordination. Over the course of the first month, we provide both in-depth assessment and ongoing support to ensure stability and momentum from day one.

Phase 1: Initial Assessment & Planning

  • Meeting 1:
    Comprehensive intake and evaluation
    (in-person or virtual; ~2 hours including travel)

  • Meeting 2:
    Review findings, provide written summary, and present 30-day service plan (in-person or virtual; ~2 hours including travel)

  • Includes:

    • Full review of care needs, goals, routines, and support systems

    • Written service plan outlining tailored strategies, care coordination needs, and recommendations

Phase 2: Ongoing Support (First 30 Days)

Hands-on, real-time support as we begin implementing your plan and adjusting based on what’s working.

This plan provides structure and peace of mind during a time when many families feel most overwhelmed. It's designed to help you move from uncertainty to clarity with a real partner at your side.

      Up to 2 hours/week for direct support, caregiver oversight, and follow-up
      8 hours total × $190 = $1,520

      Brief communication by phone, text, or video to ensure things stay on track

    • Behind-the-Scenes Coordination
      Research, referrals, documentation, and logistical care management (~30 min/week)

Care Management: 1 Hour Visit per Week

This plan is ideal for individuals who are stable but benefit from regular in-person oversight and behind-the-scenes coordination. Whether you’re managing caregivers, navigating appointments, or simply maintaining consistency, our team is here to provide expert guidance and monitoring each week.

    • Weekly In-Person Visit
      One visit per week to check in, assess well-being, oversee care, and support daily routines
      4 hours/month × $190 = $760

    • Weekly Check-Ins
      Brief phone, text, or video updates to address minor questions and maintain communication

      Behind-the-scenes coordination of care, resources, and planning (approx. 30 min/week)

Crisis Support & Stabilization

Immediate intervention and expert coordination when every moment matters.

This short-term service is designed for families facing a sudden medical, behavioral, or care-related crisis. We act quickly to assess the situation, stabilize what’s urgent, and put a plan in motion, providing both hands-on support and access to trusted resources.

Phase 1: Crisis Assessment & Immediate Action

  • Emergency Intake & Risk Assessment
    Initial in-person or virtual evaluation to understand the situation, identify risks, and determine urgent needs
    Duration: ~2.5 hours

  • Stabilization Planning
    Follow-up session to create a 7-day plan with immediate next steps, safety strategies, and referral coordination
    Duration: ~2 hours

Phase 2: Intensive Support (First 7 Days)

  • In-Person Support Visit
    Up to 2.5 hours of real-time care management, oversight, and in-the-moment problem-solving

Hospital or Facility Discharge Planning

Seamless, hands-on coordination for a safe and supported return home.

Transitions from inpatient settings, whether medical, rehab, or psychiatric, are often complex and high-stakes. This service ensures nothing is missed. We coordinate with hospital teams, secure resources, and provide in-person support through every step of the discharge and adjustment process.

    • Initial Assessment Meeting
      90-minute session to review current care status, discharge goals, home readiness, and immediate needs
      1.5 hours × $190 = $285

    • Case Management & Planning
      Behind-the-scenes work to review records, secure medications and equipment, arrange housing or home supports, and draft a post-discharge plan
      4 hours × $190 = $760

    • Medical Advocacy
      Participation in discharge planning meetings, collaboration with inpatient care teams, and advocacy for continuity of care
      3 hours × $190 = $570

    • Discharge Day Support
      Up to 6 hours of in-person coordination on the day of discharge, including transportation support, home setup, and medication tracking
      6 hours × $190 = $1,140

    • Coaching Visits (First 2 Weeks Post-Discharge)
      In-home support focused on establishing new routines, setting up services, and skill-building for independence

This plan is ideal for families who want peace of mind that every detail is covered during a transition. We stay involved through the adjustment period - not just at the hospital doors - ensuring your loved one is safe, supported, and set up for success.

How to Prepare for Your First Meeting with Us

Your consultation is an opportunity to share what’s happening, clarify your priorities, and explore whether Whitmire is the right partner for your care and life‑management needs. You don’t need to arrive perfectly organized—this guide simply helps you feel grounded and ready for a meaningful conversation.

What to Expect When You Work With Us

Families often come to us feeling overwhelmed—unsure where to start, who to trust, or what the next step should be. Our role is to bring clarity and calm to the process. Here’s what you can expect when working with Whitmire:

  • We are a private-pay firm and do not bill insurance. Our services are billed hourly, and we provide detailed estimates before beginning any engagement.

  • Invoices are sent monthly between the 1st and the 5th.

  • Our preferred payment methods are eCheck and ACH. Credit cards are also accepted with a processing fee, and you’re welcome to mail a paper check if you prefer.

    You can make a payment quickly and securely through our online payment portal. Just click the link, follow the prompts, and enter your payment details—it’s that simple!

  • Becoming a client is a simple, collaborative process. After we connect for a Discovery Call and confirm that our services are a good fit, onboarding begins:

    • The payer signs the Services Agreement.

    • The client (or their representative) signs the consent forms.

    That’s it! Once onboarding is complete, we’ll get you scheduled—and the real work (and momentum) begins.

  • We offer regular updates via phone, email, or video, depending on your preference. Many of our clients have family members or guardians in other states. Communication is a core part of our service.

  • Always. Every care plan we build is tailored to the individual’s needs, goals, and circumstances. We meet you where you are and adjust as things evolve.
